Regrettably, Lakeshore EMS, LLC has ceased operations effective Sunday, August 20, 2017.
This voluntary decision was very difficult, but unfortunately this is the action we decided needed to be taken.
We realize that this sudden action will bring many unanswered questions to our employees, vendors, facilities, and of course patients.
We wish to express our deep gratitude to the community for allowing us the opportunity to be of service to you these past few years. This is not the outcome we had hoped for, of course.

TELEPHONE CALLS: The telephone numbers for Lakeshore EMS, LLC were disconnected on November 27, 2017. There is no longer a telephone contact option.
EMPLOYMENT VERIFICATION: All mail should be directed to the address below and will be forwarded by the USPS to the alternate address or email [email protected].
PATIENTS WITH BALANCES OWED: The Lakeshore EMS, LLC Accounts Receivable Portfolio was sold to Diversified Consumer Services in August, 2017. Lakeshore EMS, LLC can not accept payment on your account. All payments must be remitted Diversified Consumer Services at 6212 US Hwy. 6, Ste. 184, Portage, IN 46368-5057. PATIENTS WITH OLDER BALANCES: If your account is not with Diversified Consumer Services or if you did not make payment or approved payment arrangements by December 31, 2017, it may have been placed for collection with Quality Asset Recovery. You can connect to them by clicking the link above. You should remit payment directly to them if this is where your account is located. Please refer to the address on your letter from them. You can contact them at 1-800-796-1476.

INSURANCE COMPANIES: If a patient has contacted you and their account is with Diversified Consumer Services, please fax written authorization from the patient to 321-256-0627. If a patient has contacted you and their account is with Quality Asset Recovery, please contact them at 1-800-796-1476 for direction.
